The three-antibiotic regimen—bedaquiline, pretomanid and linezolid (BPaL)—used for treating multidrug-resistant (MDR) pulmonary tuberculosis, may not be effective in treating TB meningitis because bedaquiline (Sirturo, Janssen) and linezolid cannot cross the blood–brain barrier, according to findings from a Johns Hopkins Children’s Center study (Nat Commun 2022;13:7974).
